On the New Form dialog box, enter the following details:<\/p>\n<ul>\n<li style=\"font-weight: 400\"><b>Name <\/b><span style=\"font-weight: 400\">&#8211; Enter a name for the form<\/span><\/li>\n<li style=\"font-weight: 400\"><b>Display Name<\/b><span style=\"font-weight: 400\"> &#8211; Enter a display name for the form<\/span><\/li>\n<li style=\"font-weight: 400\"><b>Description<\/b><span style=\"font-weight: 400\"> &#8211; Enter a brief description of the form<\/span><\/li>\n<\/ul>\n<p>4. You can create a new instance by clicking <b>Add New Instance<\/b>.
